5S Sustainment Schedule:  A form used to schedule ongoing cleaning activity in order to maintain 5S in an area until a permanent solution can be found. The form lists the regular actions that need to be taken, the frequency of activity, person responsible, and clean activity needed. Using this form allows a company and its employees time to make changes that require lead time after a 5S workshop has taken place.

5S Map: This form has two main purposes during your 5S program: to provide a current state and also document the new current state at the end of the workshop. It provides a bird's eye perspective of the area to communicate to others outside the workshop the progress taken at just a glance. Using this form assists in solidifying a strong 5S program

30 Day 5S Action Log: This form is used as the central point of communication for specific actionable items to be performed over the 30 days after the completion of a 5S workshop.
